Prepare for Your First Visit
Welcome to Feminine Forms! You’ve scheduled your first appointment with us and we can’t wait to meet you! This post will detail how to prepare your first appointment at Feminine Forms.
What should you bring?
Insurance Card: If you are planning on using insurance to get your products, we need you to bring your card (or cards, if you have a secondary insurance policy). We will make a copy of your insurance card to keep the information on file so we can file claims with your insurance.
Prescription: We will need a prescription from your doctor to file the cost of your products to your insurance company. A prescription is necessary for appointments for compression sleeves/gloves to treat or prevent lymphedema so we know the level of compression your doctor recommends. If your appointment is for other products, we can request a prescription from your doctor; we will communicate with your doctor’s office to get the information we need. However, if your doctor has given you a prescription, please bring it with you.
What should you wear?
Short answer: whatever you want!
Longer answer:
For fittings with bras and prostheses, we suggest wearing a comfortable shirt that is easy to take on and off. One of your favorite shirts is great! We want you to feel confident and beautiful as you’re trying out different prostheses. Wearing something you love and are comfortable in can also help us see how different sizes, shapes, and styles of prostheses fit on you. You are welcome to wear an undershirt, if that is normally what you wear. Usually, we recommend fitting bras and prosthesis directly to your chest wall, but we can work with whatever will make you most comfortable.
For other fittings, what you wear really doesn’t matter!
What will your first fitting look like?
You will be matched with one of our wonderful certified mastectomy fitters. Each of our fitters has gone through training to be certified for any type of fitting we offer. Either your fitter or our office manager will walk you through some paperwork. Your fitter will then talk to you so she can better understand your needs and preferences. You will have a private fitting room where she will take some measurements and then help you try on different sizes and styles of products until you find your perfect fit.
How long should you expect to spend at your appointment?
Bras and prostheses: around 60 minutes for a first fitting and 30 minutes for follow up fittings
Post-surgical camisoles: around 30 minutes
Compression sleeves: around 30 minutes
Wigs: around 30 minutes
What will you take home with you?
Ideally, you will be able to take all of your products with you after your appointment. If we don’t have your products in stock, we are able to order them and you can either pick them up another day or we can mail them to you.
We are so excited to meet you and help you find products to support you in whatever phase you are currently in. If you have any other questions before your appointment, please call us at 801-770-0600 or send us an email at info@feminine-forms.com. See you soon!
